This form must be
completely filled out and signed by parent/guardian and the
student athlete. This must be completed for every sport/season

the student athlete. This must be completed for every
sport/season.

Medical Card
This form
must be completely filled out and signed prior to any
participation, practice or try-out. This must be completed every
sport/season.

APPENDIX III (2-sided)

Medication Assistance Request
This form must be completed if your child requires
specific medical attention or assistance. This must be completed
every sport/season.

APPENDIX IV (2 pages, both 2-sided)
a.
Ohio High School Athletic Association Preparticipation Physical
Examination Form
These four pages of information must be completely
filled out prior to any participation, try-out or practice. The
doctor must sign and date the physical and the parent/guardian
and student athlete must sign and date the physical form in
order for it to be completed. This sports physical is good for
one calendar year and will be kept on file in the athletic
department, until it expires.
Please note: You must read the OHSAA Athletic Eligibility
Information Bulletin (see #7) before signing the Student
Signature and Parent or Guardian Signature found at the bottom
of the OHSAA Preparticipation Physical Examination Form.
Please note: Sports physicals are recommended to be
performed by your family physician; however, we do know that
many Berea City School District attend other medical facilities
to receive these physicals. In any case, every physical form is
accepted. We do ask that if you have a form, other than OHSAA’s,
please complete the front side of the OHSAA physical form and
attach it to your physical form.

Participation Fee
a. All fees
must be turned in prior to the first public playing event. Your
participation will be denied until payment is received. In the
event you have a financial situation preventing you from making
payment please make arrangements with the athletic department.
